Adding An Activity and Requesting Sign Off From Your Trainer

Click the Plus symbol on the top tool bar

Select the relevant form from the list you are directed to

To record an activity against outcomes in your curriculum, click ‘link to goals’

Scroll down the page to see the entire form

Select options from drop down box or enter information manually in all fields

There is an option also to add a file - You can use this to attach any relevant supporting documents

If you require sign off for this form you need to enter your trainer’s name in the field that says ‘Who would you like to fill in the next section of this form?’

Click Submit
